Colleen Cannon, MA, CDP

Colleen Cannon brings over two decades of experience supporting older adults, individuals with disabilities, and their caregivers across healthcare, behavioral health, and community-based settings. With a Master’s degree in Counseling Psychology and certification as a Dementia Practitioner, she has dedicated her career to person-centered advocacy and coordinated care.
She has held roles at the Alzheimer’s Association, Horizon Blue Cross Blue Shield of New Jersey, the Office on Aging and Disability Services, SERV Behavioral Health, and the Neighborhood Coalition for Shelter. Across these organizations, Colleen has provided dementia care consultation, managed complex caseloads, mentored care professionals, and coordinated services in both community and facility-based environments. Her work spans care consultation, case management, and behavioral health support, always emphasizing individualized care and collaborative engagement with families and providers.
Colleen is passionate about helping older individuals maintain dignity, independence, and quality of life as they age. She values working alongside the Aging Advisors team of professionals, recognized as some of the best in their field, to ensure each client’s needs are met with compassion, expertise, and continuity of care.
