Medical and Cognitive Monitoring
We develop personalized medication management programs to promote safety and and adherence. Our services include organizing and filling medications, providing education on proper use, and conducting regular monitoring to improve compliance and reduce the risk of errors.
Referrals to Trusted Professionals
We recommend reputable geriatricians, medical specialists, therapists, financial planners, elder law attorneys, adult day programs, retirement communities, hospice providers, and more.
Referrals to Government & Financial Programs
We assist with applications for Medicare, Medicaid, VA benefits, pharmaceutical assistance, and transportation services to meet health and daily living needs.
Senior Living Transition Support
We provide objective guidance on alternative living arrangement, including rehabilitation centers, assisted living, long-term care, and home- or hospital-based hospice services.
